Clinics interested in becoming PCPCH centers can participate in upcoming webinars throughout the year. A new version of the model, the 2025 PCPCH Recognition Criteria, will be implemented in January 2025. These webinars will focus on helping clinics understand the revised standards under this new version. Care Everywhere is used within the Epic Hyperspace patient record, to exchange electronic health records with outside organizations. It provides access, at the point of care, to a patient’s medical records from other organizations. Care Everywhere is a functionality within electronic health record (EHR) systems, particularly prominent in those provided by Epic Systems Corporation. It allows healthcare providers to securely share patient health information across different healthcare organizations that also use Epic's EHR system.

Here's how it typically works:

1. Consent: Before any information is shared, patients must provide consent. They can choose to opt in or out of sharing their medical records through Care Everywhere.

2. Interoperability: Epic's EHR system is designed to be interoperable, meaning it can communicate with other healthcare systems that use the same standards. This allows for seamless exchange of patient data between different healthcare providers, such as hospitals, clinics, and specialists.

3. Secure Exchange: When a patient provides consent, their health information is securely transmitted between healthcare organizations over a secure network. This ensures that sensitive patient data is protected and meets regulatory requirements, such as those outlined in the Health Insurance Portability and Accountability Act (HIPAA) in the United States.

4. Access to Comprehensive Patient Data: Care Everywhere enables providers to access a comprehensive view of a patient's medical history, including diagnoses, medications, allergies, lab results, imaging studies, and more. This comprehensive view helps providers make informed decisions about patient care, even if the patient has received treatment at multiple healthcare facilities. Access is gained through Epic or EHR that can securely share data.

5. Real-Time Updates: Care Everywhere facilitates real-time updates to patient records. For example, if a patient receives care at one hospital and then visits another facility, their updated health information is quickly available to the new provider through the shared EHR system.

Overall, Care Everywhere enhances care coordination, improves patient safety, and streamlines workflows for healthcare providers by enabling the secure exchange of patient health information across different healthcare organizations.

ECHO (Extension for Community Healthcare Outcomes) is an interactive educational and community-building experience that allows healthcare professionals throughout the state of Oregon (especially in rural and frontier regions) to create a case-based learning environment through the convenience of video-conferencing between clinical specialty experts and primary care clinicians.

The Project ECHO^© model was originally developed at the University of New Mexico. Launched in 2017, Oregon ECHO Network (OEN) is a statewide utility that supports ECHO programming determined by a collaborative group of community partners. OEN’s vision is to give Oregon’s primary care clinicians and their teams the knowledge and tools to manage health conditions that they typically refer to specialty care. The benefits of this approach include improved patient satisfaction, reduced medical errors and unneeded specialty referrals, more effective case management, and overall improved patient health for all Oregonians.
